A CVS sub-contract arrangement with Age UK Shropshire and Telford, funded by Telford and Wrekin Clinical Commissioning Group.

The Link Worker is based with colleagues within Rapid Response, working closely with Neighbourhood Officers and Care Navigators.

The role provides:

​

• Community based solutions for adults and promotes being safely cared for closer to home, seeking to reduce hospital admission.

​

The role is focused on:

​

•  Co-producing support around the needs of the person and works across many providers (both contracted and volunteer led) in the heart of where they live.

• Non-medical, supported signposting

​

The link worker is able to support you with information, advice and supported referrals into many organisations across Telford who can help you to remain cared for closer to your home. 

 

The link worker role works creatively to find healthy solutions to maintain your health and well-being.
